Jordan Brown a5c498e0f7 Various things 
- Remove lagacy user/pass header auth
- Use .WithToken on octokit calls
- Cleaned up ApiHeader validation failures and responses
- ApiController constructor parameter requireHeaders now defaults to true
- Added integration test for a bunch of 4XX errors
